PreferredOne
PreferredOne cardholders may be eligible to participate in the FitnessAdvantage membership dues reimbursement program. Check with your employer or on PreferredOne.com to determine if your benefit plan is eligible.
For those eligible, the standard program is to work out 12 or more times per calendar month to receive credit on your reported monthly dues - not to exceed $20 credit per month. One covered dependent (must be 18 years or older) may also enroll. Some customized programs are also offered, so check with your employer for details.
Medica
Medica subscribers in an eligible plan can receive up to a $20 credit on a Life Time membership each month through the Medica Fit Choices program. You must work out a minimum of 12 days each month.
All Adults (18+) covered by an eligible Medica plan may enroll in the program. You will receive up to a $20 credit toward your monthly membership dues for meeting the required number of visits. Check with Medica customer service to verify if your plan is eligible. The qualifying adult within may vary by month, but only one credit applies per membership.
Note: Some Medica groups are not eligible for this program. Some plans may have different minimum requirements.
Health Partners
HealthPartners members in eligible plans can sign up to participate in the Frequent Fitness program and receive a $20 monthly credit on membership fees. You must work out a minimum of 12 days each month.* Life Time memberships can receive credit for up to two adults participating in the program for a total credit of $40 per month.*
*Some HealthPartners groups are not eligible for this program. Some HealthPartners plans may have different minimum requirements and eligibility. Please check with your employer if you have questions about your eligibility for the program.
Blue Cross and Blue Shield of Minnesota
Eligible Blue Cross and Blue Shield of Minnesota members have a Blueprint for Health fitness discount icon on their insurance membership cards. Bring your card to Life Time Account Services to sign up and receive the $20 monthly credit. You must work out a minimum of 8 or 12 days each month depending on your plan. Life Time memberships can receive credit for up to two adults participating in the program for a total credit of $40 per month.

United Healthcare
UnitedHealthcare’s® Fitness Reimbursement program rewards eligible UnitedHealthcare plan members each month they visit and check into a participating facility the number of times required by their plan. Members are rewarded based on options chosen by their employer. Number of required visits may vary among employers. Humana
Life Time members may be eligible for Humana’s Go365 wellness and rewards program by registering their Life Time member ID on the Go365 website:
