By clicking “Submit” below, I consent to the continuation or resumption (as applicable) of my relationship with Life Time with the Membership Type, at the Center, and at the dues rate(s), specified above. With respect to my current One-Day Membership and resuming with each additional purchase and redemption of a One-Day Membership, I agree that the terms and conditions of my most recent General Terms Agreement, Member Usage Agreement and Digital Membership Terms apply.  With respect to this Membership, I authorize Life Time or its agents to automatically and without notice (1) charge or debit, as applicable, my Monthly Payment Method, my Club Tab Account or such other payment method accepted or permitted by Life Time to pay all Financial Obligations, including without limitation my Dues and Fees, whether recurring or otherwise, taxes and surcharge fees, Payment Related Fees and (2) to the extent I qualify under terms and conditions in Life Time’s sole discretion, apply a credit against such Financial Obligations in the amount I paid for my One-Day Membership(s).  Furthermore, I acknowledge that Membership levels, types, amenities, services, programs may vary by location and are subject to change, and that access to other clubs, or services, programs, amenities, or areas within a club (e.g., outdoor pools, racquet courts) may be restricted, conditioned, or otherwise limited by specific terms and conditions and/or subject to a separate or supplemental fee for access or use (e.g., your access to the outdoor pool area may require a separate fee, and be limited to your designated club). In addition, I agree that a One-Day Membership dues credit towards a new monthly membership must be redeemed within 30 days of the date I use the One-Day Membership.
